The Effect of Clonidine on Respiratory Compliances during Laparoscopic Cholecystectomy

BACKGROUND: We evaluated the effects of clonidine on respiratory compliances and hemodynamic changes during laparoscopic cholecystectomy.
METHODS: 45 patients scheduled for laparoscopic cholecystectomy were randomly allocated to three group: Control group (placebo group, n = 15), Group 1 (clonidine 1 microgram/kg, n = 15), and Group 2 (clonidine 2 microgram/kg, n = 15). Peak airway pressure (Ppeak), plateau pressure (Pst), and esophageal pressure (Pes) were measured. The compliance of the static respiratory (Cst,rs) and chest wall (Ccw) were obtained by dividing the tidal volume by airway pressure at end-expiration-Pst and Pes at end-expiration-Pst,es, respectively. The static lung compliance (Cst,L) was obtained from the Cct,rs and Ccw. In all experiments, measurements were performed at the same following times, with the patient in the reverse Trendelenburg position: PreI; 10 min after induction of anesthesia, before CO2 insufflation; I; 1 min after CO2 insufflation; 15 min; 15 min after CO2 insufflation.
RESULTS: The compliance of dynamic respiratory system (Cdy,rs), static respiratory system (Cst,rs), static lung (Cst,L), and chest wall (Ccw) in all groups were smaller at 1 and 15 min after CO2 insufflation than before CO2 insufflation (P < 0.05). In group 2, Cst,L was greater at 1 and 15 min after CO2 insufflation than in the control group (P < 0.05). Also Ccw in group 2 was greater at 1 min after CO2 insufflation than control group (P < 0.05). Mean arterial pressure in group 1 and 2 were significantly lower at 1 min after CO2 insufflation than control group (P < 0.05).
CONCLUSIONS: We conclude that clonidine administration may attenuate the significant decrease in respiratory compliances during laparoscopic cholecystectomy without significant hemodynamic changes.
